Isaac Success put Watford ahead early in the second half before Christian Kabasele was sent off for fouling Dele Alli, who scored the resulting penalty.
Erik Lamela chipped Heurelho Gomes to put Spurs ahead, but Etienne Capoue's deflected shot took it to penalties.
Tottenham goalkeeper Paulo Gazzaniga saved twice in the shootout before Alli scored the winner.
Although Spurs were the 'home' side, the game was held in Buckinghamshire because of delays in the construction of Spurs' new 62,000-seater stadium.
